Outcomes-Based Care Manager

Remote - US

CentralReach is a leading provider of autism and IDD care software for Applied Behavior Analysis (ABA), multidisciplinary therapy, and special education. Trusted by more than 200,000 users, we enable therapy providers, educators, and employers to scale the way they deliver ABA and related therapies with innovative technology, market-leading industry expertise, and world-class customer satisfaction.

The Outcomes-Based Care Manager is a customer-facing clinical expert who helps provider organizations shift from measuring care by hours delivered to measuring it by outcomes achieved. You will work directly with customer clinical and executive leaders to implement outcomes frameworks, interpret progress data, and build the evidence that supports value-based and outcomes-linked payer relationships. You will use CentralReach's platform and AI-enabled tools to make that work scalable, and you will bring what you learn from the field back to our Clinical and Product teams. This is an individual contributor role reporting to the VP, Payor Strategy and Network Development.

Key Responsibilities

Customer Advisory and Engagement

  • Serve as the clinical outcomes expert for assigned customers, advising clinical directors, operations leaders, and executives on adopting outcomes-based care models
  • Lead outcomes-focused onboarding, implementation, and optimization engagements, including discovery sessions, workshops, and recurring business reviews
  • Assess each customer's current data practices and outcomes maturity, then build a practical roadmap to close the gaps
  • Deliver training and best-practice guidance that helps clinical teams apply outcomes measurement in daily workflows

Outcomes Measurement and Clinical Insight

  • Apply standardized outcome measures and benchmarks across ABA and IDD service lines, adapting them to each customer's population and payer mix
  • Review customer treatment progress trends, flag at-risk caseloads, and recommend data-informed adjustments to care approaches
  • Help customers configure CentralReach reporting and dashboards so they reflect clinically sound outcomes methodology

Payer and Stakeholder Readiness

  • Help customers build outcomes reports and narratives for payers, funders, and families
  • Advise on preparing for value-based and outcomes-linked arrangements, including the quality metrics payers are likely to ask for
  • Stay current on payer expectations, quality measures, and regulatory developments in autism and IDD care

AI-Enabled Care and Continuous Improvement

  • Coach customers on using AI-enabled features responsibly for analysis, documentation, and insight generation, always grounded in clinical judgment and ethical oversight
  • Capture customer feedback and field insights and share them with Product, Clinical, and Data teams to shape the roadmap
  • Contribute to reusable assets such as playbooks, templates, and case studies so outcomes expertise scales beyond one-to-one engagements

Cross-Functional Collaboration

  • Partner with Customer Success, Sales, Product, and Data teams to align outcomes work with customer goals and clinical integrity
  • Support pre-sales and renewal conversations with clinical credibility, and represent CentralReach in webinars, conferences, and thought leadership as appropriate

Qualifications

Required

  • Active BCBA certification in good standing, or an equivalent clinical credential in autism/IDD care
  • [4+] years of clinical experience in ABA or IDD services, including supervisory, clinical leadership, or quality-focused work
  • Experience advising, training, or consulting with external clients or stakeholders
  • Strong understanding of clinical data collection, progress monitoring, and how treatment data translates to outcomes
  • Ability to interpret data and communicate findings clearly to both clinical and executive audiences
  • Comfort with software platforms and data tools, and curiosity about applying AI to clinical and operational work
  • Ability to manage multiple customer engagements independently

Preferred

  • Experience with value-based care, outcomes-based contracting, or payer quality programs
  • Familiarity with CentralReach or comparable practice management and EHR platforms
  • Background in program evaluation, quality improvement, or clinical analytics
