Centria Healthcare is seeking a compassionate and dedicated Registered Nurse (RN) or Licensed Practical Nurse (LPN) to provide one-on-one care for an adult male client in his smoke-free home in Sunfield, MI. This is a day shift position offering the chance to step away from the fast-paced hospital environment and form a meaningful connection with a single patient.

About the Client:

• A high-functioning MVA survivor who requires support with medication compliance and basic care needs.
• Enjoys spending time outdoors, playing with his small dog, and taking walks.
• Actively involved in his church community, attending services twice a week.
• Requires nursing assistance for medical appointments and outings.

Key Responsibilities:

• Provide skilled nursing care based on the patient’s plan of care.
• Assist with medication management and basic daily care needs.
• Support TBI and SCI rehabilitation.
• Accompany the client to medical appointments and outings.
• Monitor and document physical, mental, and emotional well-being.
• Communicate effectively with the clinical team to ensure optimal care.

Qualifications:

• Active RN or LPN license in Michigan.
• Strong communication skills (written and verbal).
• Ability to follow care plans and documentation protocols.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• Negative TB test or chest X-ray.
• Current flu vaccination.
