Position Overview:
The Physical / Occupational Therapist provides home-based services to clients across the state of Michigan. The individuals we serve receive services for injuries subsequent to MVA primarily. Consistent weekly visits, typically one to two times per week.
Responsibilities:
The ideal candidate must be able to perform the following essential job functions with or without reasonable accommodations:
- Provides skilled physical / occupational therapy in accordance with the physician’s plan of care. Accepts only patients referred by the patient care manager or designee.
- Assists the physician in assessing the patient’s functional level by applying appropriate tests and assists in the development and revision of the plan of care, including OASIS assessments and goal setting as appropriate.
- Treats patients to relieve pain, develop and restore function and maintain maximum performance, using appropriate physical therapy treatments, modalities and procedures as stated in the physical / occupational therapy plan of care. Identifies barriers to desired outcomes.
- Observes, records and reports the patient’s reaction to treatment and any change in the patient’s condition to the physician and/or Client Service Manager and other staff involved.
- Instructs and advises patient, family and other health team personnel including, when appropriate, the physical therapy assistant / occupational therapist assistant and home health aides, in the phases of physical / occupational therapy in which they may assist the patient; instructs all of the above in the use of prosthetic and orthotic devices and durable medical equipment, as appropriate.
- Provide quality therapy services to clients and initiate referrals when appropriate in accordance with established standards of practice, therapy department procedures, and productivity standards
- Evaluate clients to obtain data necessary for treatment planning and implementation
- Develop treatment plans by identifying a target list, including short and long-term goals and methods to achieve identified goals
- Follow all applicable policies and procedures as well as current federal, state, and local standards, guidelines and regulations that govern long-term care
Required Qualifications, Skills, and Knowledge:
- Degree from an accredited Physical / Occupational Therapy Program
- Current professional licensure as a Physical Therapist or Occupational Therapist license eligible based on the rules and regulations of the state in which you are applying for role
- Current CPR Certification
- State licenses must be maintained
- All compliance standards must be completed as requested
- Strong written and oral communication skills, as well as good organizational ability
- Strong interpersonal skills with the ability to exercise patience, caring and concern in new and sometimes stressful situations
- Computer literacy, with a proficiency in standard business software applications (i.e., word processing, email and spreadsheets)
Working Conditions:
Centria’s office hours are Monday through Friday from 8:30am-6:00pm. Additional time may be required to complete above work or meet company objectives.
Physical Requirements:
While performing the duties of this job, physical requirements such as bending, reaching and lifting of up to 50 pounds may be required. This role will require sitting a majority of the day as well as walking as standing when needed. This role will require close visual activity on multiple computer screens or monitors and the ability to analyze data and figures on a screen.
