Home Health Aide / CNA – Elder Care in Rockford, MI
Job Description:
We are seeking 3-4 compassionate and reliable Home Health Aides or CNAs to provide quality care for an 82-year-old kind, quiet gentleman with multiple fractures (vertebra, pubis, ribs, shoulder, and collarbone), overall weakness, and difficulty walking. The primary focus will be on safety and fall prevention while assisting with daily tasks.
Schedule & Location:
- Location: Rockford, MI (Client resides with his girlfriend)
- Shifts:
- Mondays & Tuesdays: 10:00 AM – 10:00 PM
- Wednesdays & Thursdays: 10:00 AM – 6:00 PM
Key Responsibilities:
- Patient Safety:
- Ensure the client's safety at all times with a focus on fall prevention and careful mobility assistance.
- Daily Living Assistance:
- Assist with personal care tasks as needed.
- Help with folding clothes, washing dishes, and meal preparation.
- Home Environment:
- Maintain a safe and organized home environment.
- Work alongside the client's girlfriend to ensure comprehensive care.
- Companion Care:
- Provide empathetic and respectful support.
- Interact kindly with the client's 14-year-old, super-sweet English Bulldog (approximately 80 lbs).
Qualifications:
- Previous experience as a Home Health Aide or Certified Nursing Assistant preferred.
- Strong understanding of fall prevention strategies and safe patient handling.
- Reliable, patient, and compassionate with a proven ability to work with elderly individuals.
- Ability to work collaboratively with family members and other care providers.
- Must be comfortable around pets, as there is one senior English Bulldog in the home.
Other Responsibilities:
- Assist with mobility and transfers to prevent falls.
- Provide basic personal care, including hygiene, toileting, and dressing.
- Monitor and assist with oxygen therapy as needed.
- Prepare light snacks or drinks if requested.
- Offer companionship and reassurance throughout the shift.
- Assist in positioning and turning to prevent discomfort.
- Document and report any changes in condition.
- Respond to any emergencies promptly.
- Perform duties outlined in patient's plan of care
- Monitor patient condition by observing physical and mental condition, intake and output, and exercise
- Observe the client’s physical, mental, and emotional conditions/ document and record changes in the patient condition
- Maintain a safe, secure, and healthy patient environment
- Assist with daily living activities, such as dressing or taking a bath
- Adhere to professional standards, facility care policies and procedures, federal, state, and local requirements
Qualifications:
- CNA, HHA, or equivalent experience in home care preferred.
- Experience working with fall-risk patients and mobility assistance is highly desirable.
- Ability to assist with personal care and provide compassionate support.
- Must be reliable, patient, and attentive to the client’s needs.
- Comfortable working in a private home.
- Ability to work independently and follow care plans.
- Excellent oral and written communication skills
- Ability to follow verbal and written instructions
- Ability to utilize computers/tablets and computer software
- Strong patient service skills
- Attention to detail and ability to carefully follow protocols and rules regarding the patient's care
- Ability to address minor medical issues

Documentation requirements:
- Valid Driver's License
- Valid CPR Certification
- Background Check
- Negative TB test results
- Auto Insurance with your name listed on the policy
