
Home Health Occupational Therapist (Per Visit)

Job Summary:
We are seeking a compassionate and skilled Occupational Therapist to provide in-home therapy services to patients across various regions of Michigan. This per-visit position offers flexible scheduling and the opportunity to make a direct impact on patients' independence and quality of life in their home environments. The ideal candidate is experienced in home health, self-motivated, and comfortable working independently. A significant portion of the caseload includes individuals recovering from motor vehicle accidents (MVA), requiring a strong understanding of trauma, orthopedic injuries, neurological impairments, and chronic pain management.
Key Responsibilities:
- Evaluate patients’ functional abilities and develop individualized treatment plans tailored to MVA-related injuries and conditions.
- Deliver occupational therapy services in patients’ homes, following physician orders and home health care regulations.
- Instruct patients and caregivers on safe and effective techniques for daily living activities, adaptive equipment, and energy conservation.
- Complete and document visits thoroughly and promptly using electronic medical records (EMR).
- Communicate with the interdisciplinary care team to ensure coordinated care.
- Educate patients and families to promote independence and prevent injury or decline.
- Re-evaluate and adjust therapy plans as needed based on patient progress and physician input.
- Ensure compliance with all federal, state, and agency regulations.
Qualifications:
- Licensed Occupational Therapist (OTR/L) in the State of Michigan.
- Graduate of an accredited Occupational Therapy program.
- Minimum of 1 year of OT experience preferred; home health and/or experience with MVA clients highly desirable.
- CPR certification (BLS).
- Valid driver’s license, reliable transportation, and auto insurance.
Schedule and Compensation:
- Per visit / PRN schedule — flexible hours based on patient needs and your availability.
- Competitive per-visit pay based on experience and location.
Additional Information:
- Service areas may vary based on your location in Michigan. Travel is required within assigned territory.
- Orientation and clinical support provided.

Working Conditions:
Centria’s office hours are Monday through Friday from 8:30am-6:00pm. Additional time may be required to complete above work or meet company objectives.
Physical Requirements:
While performing the duties of this job, physical requirements such as bending, reaching and lifting of up to 50 pounds may be required. This role will require sitting a majority of the day as well as walking as standing when needed. This role will require close visual activity on multiple computer screens or monitors and the ability to analyze data and figures on a screen.
