Job Title: Registered Nurse Case Manager

Job Summary
We are seeking a compassionate and dedicated Registered Nurse Case Manager to provide high-quality patient care in accordance with physician orders, hospice standards, and company policies. In this role, you will collaborate with an interdisciplinary care team to support patients and families while ensuring the best possible clinical outcomes and quality of life.

We are currently offering a $5,000 sign-on bonus for this position.

Key Responsibilities

  • Conduct initial and ongoing assessments of patients and families to determine hospice care needs

  • Provide skilled nursing care in the home setting using the nursing process

  • Develop, implement, and update individualized care plans based on patient assessments

  • Coordinate and deliver interdisciplinary hospice services in collaboration with the care team

  • Complete accurate and timely documentation within 24 hours of each patient visit

  • Administer medications and treatments as prescribed by the physician

  • Educate and support patients and families on care techniques, symptom management, and disease progression

  • Serve as the primary Case Manager for assigned patients, coordinating care and managing caseloads

  • Communicate patient status, needs, and changes with physicians and members of the healthcare team

Education and Experience

  • Graduate of an accredited school of nursing; BSN preferred

  • Minimum of one year of professional nursing experience required

  • Hospice or home health experience preferred

  • Experience caring for aging, chronically ill, or palliative care patients is highly desirable

Certifications and Requirements

  • Current RN license in Arizona or compact state

  • Valid driver’s license and reliable, insured transportation

  • Current BLS certification from the American Heart Association or American Red Cross

Physical Demands and Working Conditions

  • Ability to stand, walk, sit, bend, and perform routine physical tasks

  • Ability to lift up to 50 pounds

  • Frequent travel to patient homes, typically completing 1 to 10 visits per day in varying weather conditions

  • Proficiency in electronic documentation using a tablet or laptop

Skills and Competencies

  • Strong time management and organizational skills

  • Excellent communication and interpersonal abilities

  • Strong clinical observation and nursing judgment

  • Ability to work effectively in a fast-paced and sometimes stressful environment

  • Knowledge of hospice care practices and regulatory standards

Hourly range for posted region

$38 - $48 USD
