Licensed Practical Nurse (Referral Management)
Coho Integrated Solutions is seeking a compassionate and detail-oriented Licensed Practical Nurse (Referral Management) to support healthcare operations at Alexander T. Augusta Military Medical Center in Fort Belvoir, Virginia. The LPN will manage Clear Legible Report (CLR) Services, ensuring efficient referral management, data accuracy, and seamless coordination with healthcare providers, administrative staff, and leadership.
Responsibilities (include but are not limited to):
- Manage all aspects of Clear Legible Reports (CLRs), including downloading/uploading from electronic records, notifying providers of abnormal results, closing referrals, and tracking/reporting outcomes.
- Oversee front desk operations and ensure data quality management to support smooth clinical workflows.
- Receive, review, and determine disposition of all CLRs by interpreting referring consults, physician notes, and treatment protocols to correctly associate reports with patient encounters.
- Monitor and follow up on tests, referrals, and results to ensure timely physician notifications and accurate documentation in accordance with quality assurance standards.
- Generate recurring and ad-hoc reports to identify trends, support decision-making, and enhance health information management practices.
- Collaborate with leadership to evaluate the effectiveness of the CLR process and recommend improvements ensuring compliance with access standards and operational guidelines.
- Utilize CHCS and AHLTA systems to review and close out network referrals, ensuring care was rendered within authorized periods.
- Screen laboratory, radiology, and clinical test results to ensure timely notification of abnormal findings to providers.
- Provide regular updates to supervisors on CLR productivity, performance metrics, and workflow optimization.
- Address informal staff inquiries, track operational challenges, and report performance feedback to management for continuous process improvement.
Essential Job Qualifications and Requirements:
- Citizenship:
- Proof of US Citizenship is required to work on this contract.
- Education and Certifications:
- Graduate of an accredited Licensed Practical Nurse Program (State-accredited).
- Active, unrestricted LPN license in a U.S. state, District of Columbia, or U.S. territory.
- Basic Cardiac Life Support (BCLS), certified by the American Society of Clinical Pathologists (ASCP) or Health and Human Services Medical Certification.
- Experience and Skills:
- Minimum of two (2) years of experience in a clinical healthcare environment.
- Practical knowledge with Zimbra, CHCS, AHLTA, and HAIMS systems.
- Strong analytical, organizational, and communication skills with a commitment to accuracy and patient confidentiality.
