Independent Practice Association (IPA) Registered Dietitian - Interest Form

If you are an RD with an existing private practice who wants to focus your energy on delivering MNT to your patients and avoid the additional, related administrative tasks like billing, credentialing, tech, EMR, etc., feel free to submit your information in the form below.  Culina will review your information and let you know if you are eligible to participate in its IPA.

DISCLAIMER: You will be a 1099 independent contractor of Culina, not an employee.  Under this arrangement, you will (i) provide all professional clinical services within your scope of practice, (ii) assume responsibility for all medical decision-making, and (iii) exercise your independent judgment as a licensed professional. Culina will only provide administrative support.

Functions:

• Provide exceptional, culturally competent, evidence-based medical nutrition therapy to a diverse patient population
• Utilize the Healthie electronic medical record system, including documenting in the ADIME format
• Complete comprehensive nutrition assessments, including nutrition history, lifestyle history, stage of change, and barriers to change
• Utilize evidence-based nutrition interventions to guide patient education and care
• Collaborate with patients to create personalized SMART goals and strategies
• Maintain strict compliance with HIPAA
• Completing billing for patients
• Promote the Culina Health core values
• Maintain RD credential
• Maintain appropriate insurance

Requirements:

• At least 2 state licenses: CDN, CD, LD and/or LDN
