ABOUT THE ROLE 

As one of Daymark’s first APPs, you’ll work with our clinical operations team to implement our cancer care model. You’ll care directly for patients and enable our care team of RNs, community health workers, and social workers. Your primary goal within the next 18 months is to provide great patient care by implementing and helping continuously improve the SOPs and processes that will enable Daymark to be effective in our first market and expand into new markets, build a highly engaged patient population, and form a high-performing clinical team. You must be comfortable operating as a player/coach, rolling up your sleeves, and doing whatever is required to provide high-quality clinical care and to grow quickly. You’ll work closely with our clinical operations team, leveraging your expertise to establish our evidence-based oncology care and navigation model to address clinical (symptom care, palliative care, mental and behavioral health care) and non-clinical (social and related needs).   

WHAT YOU’LL DO 

  • Provide clinical care: You’ll provide direct patient care predominantly via virtual platforms and will support our in-home intake process.  You’ll conduct assessments, place orders, prescribe medications, document appropriately, and assign tasks to enable our broader care team to deliver great patient care.     
  • Enable Daymark’s ability to triage for high acuity: You’ll help us provide navigation and direct clinical care for our highest acuity patients while also supporting our broader patient population. You’ll collaborate closely with oncology practices in our market through daily standups with their own clinical leadership teams.  
  • Translate SOPs into real-world application: You’ll collaborate with Daymark’s clinical operations team to refine and implement our SOPs that drive our evidence-based oncology care and navigation model. You’ll also help us build out care pathways to stratify our patient populations, translating it into our product, and ensuring we can get all patients in our care the right treatment at the right time. 

WHAT WE ARE LOOKING FOR 

  • Expertise: You are an active NP or PA, have been practicing for at least 4 years, and have experience providing direct clinical care to patients with cancer. You’ve had experience tailoring or improving care models and launching them in an organization or market.  Oncology Nursing Society certification, experience with value-based care and/or in home care are each a plus.  
  • Tech savvy: You are comfortable caring for patients with cancer both in-person and virtually over video and web-based call software.   
  • Analytical Skills: You can roll up your sleeves and contribute to clinical SOPs and educational materials in collaboration with your clinical operations counterparts. 
  • Biased to action: You know how to identify and prioritize your initiatives and ensure that urgent and important tasks always get done. 
  • Comfort with ambiguity: You have experience being a part of building or launching new programs or processes and understand that rapid changes to the business, strategy, organization, and priorities are par for the course. 
  • Player/Coach: You can lead your team and can care for patients and switch contexts with ease, as all will be required in a day’s work. 
  • Able to work in-person. You'll be working primarily in Rhode Island, but may work with the team in Connecticut and the Greater Boston Area.

Compensation Range:
$140,000 - $170,000 USD
