Eliot is seeking Intensive Care Coordinators to provide community based care coordination. This role works with Eliot’s Children’s Behavioral Health Initiative (CBHI) program, a program that supports children with behavioral, emotional and mental health needs and their families with integrated behavioral health services and a comprehensive community-based system of care.
As an Intensive Care Coordinator, you will support youth and family by coordinating services, care team members and collateral supports working in concert with the youth, family to provide education and advocacy.
Responsibilities:
- Conduct outreach to new referrals, and initiate intake meetings
- Guide families through the Wraparound process, assess strengths and needs.
- Complete comprehensive home-based assessments and coordinate the development of safety plans and Individual Care Plans (ICPs) with families and collaterals
- Coordinate services, make referrals, support system navigation, and help build natural family and community supports
- Facilitate monthly Care Planning Team meetings, monitor services, authorize supports, and participate in rotating on-call coverage
Qualifications:
- Requires a Bachelor’s degree or Master's degree in Human Services or related field
- Experience working with youth and family as well as system and resource navigation
- Valid Massachusetts Drivers License
- Bilingual in English and either Spanish, Haitian Creole, Portuguese, Mandarin, or Arabic preferred
Schedule: 40 hours/week. Hybrid and Flexible schedules available.
Annual Salary
$47,000 - $60,000 USD
