Eliot is seeking motivated Bilingual (Spanish) Care Managers to join our Community Behavioral Health Center (CBHC) which serves as a single entry point for mental health and substance use services for youth, families and adults. The ideal candidate will provide in-home support and services to clients, primarily focusing care coordination and ensuring that treatment goals are culturally competent, realistic, attainable and promote recovery.
Responsibilities:
- Provide client support focusing on problem-solving, skills training, behavior modeling, optimism, and encouragement
- Deliver interventions in the office, virtually, in the community, and at clients' residences based on client preferences, needs, and clinical direction
- Enroll eligible individuals in care management programs as needed.
- Assist the team and clients in ongoing assessment of symptoms and responses to interventions
- Contribute to the development of treatment plans as part of the client's treatment team
- Ensure treatment goals are culturally competent, strength-based, realistic, attainable, and promote recovery
- Ensure each individual is actively involved in their treatment planning process, and their preferences are incorporated
- Coordinate and ensure timely face-to-face contact with enrollees in the community following inpatient discharge.
Qualifications:
- Bachelor’s degree in Psychology, Social Work or related field
- 3 year experience working with adults with mental illness and/or substance abuse
- Fluent in Spanish and English
- Valid Massachusetts Driver's License
Schedule: Monday-Friday, 9am-5pm with remote/hybrid flexibility(3 days on-site, 2 days remote)
Annual Salary
$53,000 - $53,000 USD
