Eliot is seeking a Service Navigator to join our Community Behavioral Health Clinic (CBHC) which serves as a single entry point for mental health and substance use services for youth, families and adults. As a Service Navigator, you will be the first point-of-contact providing triage support to individuals and families experiencing acute distress as a result of mental health and co-occurring disorders thereby requiring services through CBHC. You will also oversee the warm handoff process and develop short term stabilization treatment plans for each individual and/or family.
Responsibilities:
- Provide triage and community-based supports including crisis evaluations, short-term stabilization, and assistance with maintaining community stability
- Complete diagnostic assessments and develop short-term treatment plans to address immediate needs during the transition to a CBHC team
- Deliver side-by-side interventions focused on problem solving, skills training, and promoting stability for individuals and families
- Participate in team huddles, meetings, and warm handoffs by sharing key assessment and treatment planning information
- Ensure treatment goals are culturally responsive, strength-based, recovery-oriented, and achievable
Qualifications:
- Requires a Master’s degree in Psychology or related field
- Experience working with adults with severe mental illness
- Valid Massachusetts Drivers License
- Licensure from the Commonwealth of Massachusetts is preferred (LICSW, LMHC,LCSW, LMFT, RN)
Schedule: Monday-Friday, 9am-5p or 10am-6pm
Annual Salary
$70,000 - $80,000 USD
