Eliot's Family-based Intensive Treatment (FIT) services will offer short term intensive treatment for higher acuity children, youth and families for whom more traditional outpatient services may not be effective or accessible. A FIT Team is composed of highly trained Clinicians, Care Managers and Family Partners, that will work closely with a family at least three times each week, including individual, caregiver and family sessions, skills coaching and care coordination support. The FIT Team will be trained to deliver innovative and evidence-based clinical care anchored in Values Based Transdiagnostic treatment.
The ideal candidate for the Family-Based Intensive Treatment (FIT) Family Partner will be a compassionate and highly organized professional with lived experience supporting families through intensive, community-based behavioral health services. They will demonstrate strong skills in collaboration, crisis intervention, and care coordination while empowering caregivers to navigate complex systems and meet treatment goals.
Responsibilities:
- Engage in all face to face intake and assessment related activities including collection of information from the family and any/all involved providers and collaterals to be utilized in a comprehensive values-based strength and needs assessment.
- Facilitate and/or co-facilitate individual and family sessions, team, collateral, provider and care planning meetings.
- Engage caregiver(s) in activities in the home and in the community that are designed to address one or more goals/tasks on the family’s treatment plan.
- Teach caregiver(s) how to network/link to community resources and treatment providers.
- Support parent/caregiver in meeting the needs of youth through educating, supporting, coaching, modeling and guiding, utilizing the empowerment model.
- Provide interventions related to evidence-based treatments for families with the context of emergency response/evaluation and as part of the FIT team.
- Support caregiver(s) in advocating for services and resources, navigating systems, and engaging in proactive and responsive communication and coordination to meet youth and family needs.
Qualifications:
- Bachelor’s degree in a human services field and three (3) or more years’ experience working with children, adolescents or transition age youth
- Or, High School diploma/GED with five (5) years’ experience.
- Must be a parent, guardian or caregiver of a youth with special needs, preferably behavioral health needs.
- Experience in navigating child and family service systems and teaching and advocating for families who are involved in those systems.
- Must have valid driver’s license
Schedule: Hours of program are 8am-8pm. 40 hours per week schedule to include some evening hours and participation in a rotation of weekend coverage and after hours on-call.
Annual Salary
$28.85 - $28.85 USD
