POSITION OVERVIEW
Patient Care Coordinators serve as compassionate and reliable partners in the care journey—providing a warm, welcoming, and respectful first impression to our patients and families. As a vital member of the care team, you will engage with patients during check-in, registration, and throughout their visit, ensuring they feel heard, valued, and supported.
This role is more than administrative—it’s a key part of our culture. Patient Care Coordinators help build a positive, inclusive environment by demonstrating empathy, accountability, teamwork, and strong interpersonal communication. You’ll collaborate closely with clinical teams, contribute to seamless operations, and support efforts that improve patient outcomes and experiences.

WHAT YOU’LL DO:
- Create a welcoming, respectful, and inclusive environment for all patients and families by consistently demonstrating empathy, active listening, and genuine care.
- Guide patients through check-in and registration processes with clarity, warmth, and patience.
- Schedule appointments, consults, and referrals while clearly communicating next steps with patients and collaborating with clinical teams to ensure efficiency and coordination.
- Serve as a dependable and accessible resource for patients, families, team members, and clinicians — proactively addressing questions, resolving issues, and following up with care.
- Communicate effectively with clinicians and team members to support continuity of care and a smooth patient experience.
- Support clinic operations through adaptability, teamwork, and a shared commitment to excellence; responsibilities may include cross-coverage, participation in special projects, or process improvements.
- Accurately collect and update patient demographic, insurance, and contact information in the electronic medical record (EMR).
- Support administrative workflows such as managing calls, handling scheduling requests, processing medical documents, and preparing for patient visits.
- Partner with care teams to support outreach efforts related to follow-ups, gaps in care, and quality metrics.
- Assist with patient intake and rooming when needed, including obtaining and documenting vital signs.
- Uphold clinic standards by maintaining a clean, organized, and professional environment and adhering to opening/closing routines.
- Model a team-first mindset—collaborate with peers, support training of new staff, and contribute to a positive and respectful work culture.
WHAT YOU’LL NEED:
- High school diploma or GED preferred; candidates with equivalent years of experience in a similar role will also be considered.
- 1+ year of in a patient-facing, customer service, or community-centered role.
- 1+ year of experience using basic computer systems (e.g., Microsoft Office Suite); comfort navigating electronic health/medical records or similar scheduling systems.
- Familiarity with medical terminology and healthcare processes is strongly preferred.
- Demonstrated ability to build rapport, maintain trust, and communicate with empathy across diverse populations.
- Experience working with children and young people is highly valued.
- Strong organizational skills and ability to manage multiple priorities with attention to detail and care.
- Clear, respectful communication—both written and verbal—especially in emotionally sensitive situations.
- Ability to remain composed and collaborative in fast-paced or high-pressure settings.
- A commitment to continuous improvement, ownership of responsibilities, and a culture of mutual respect.
