Bilingual Nurse Navigator, Oncology
Location: Dallas TX, Hybrid (3 days in-office, 2 days from home)
Reports To: Director of Case Management
Job Overview:
The Level 1 Oncology Nurse Navigator plays a vital role in guiding and supporting patients and their families through the entire cancer care continuum, from navigating screening for a possible diagnosis through treatment and into survivorship. This Nurse serves as a consistent point of contact, providing evidence based, compassionate, and holistic support while helping patients understand their diagnosis, treatment options, and the care pathway ahead. Through education and emotional support, they empower patients and families, ensuring informed decision-making and greater confidence in navigating the complex healthcare system.
In this multifaceted role, the nurse navigator coordinates multidisciplinary care, ensuring timely appointments, seamless communication among healthcare providers, and access to community resources. They monitor patient outcomes, contribute to quality improvement initiatives, and delegate specialized needs to appropriate support staff. By collaborating closely with the broader care team and aligning patient goals with clearly defined services, the Oncology Nurse Navigator significantly enhances the patient experience and fosters better clinical outcomes throughout the cancer journey.
Responsibilities:
- Establish and maintain high quality meaningful relationships with patients so they feel supported and want to engage with you
- Guiding and supporting patients/families throughout the cancer care and survivorship experience.
- Oversees, directs, and provides holistic, culturally competent, and evidence-based nursing care.
- Monitors patient outcomes and participates in quality improvement projects.
- Contributes to and collaborates with healthcare team members to positively impact patient outcomes and patient experiences.
- Assists patients or their caregivers with developing scope of services with clearly stated goals and objectives.
- Providing education and support to the patient and family
- Identifying special needs of the patient and delegating to appropriate support staff
- Ensuring understanding of treatment options available
- Facilitating patient care plan recommended by physician
- Connecting patient and family with community resources
- Coordinating multidisciplinary care from time of diagnosis throughout treatment
- Improving timeliness of appointment
Requirements:
- Bachelor's degree in nursing
- Excels at building high-quality trusting relationships with members
- 1-3 years of nursing experience
- Strong communication skills
- Ability to work in a team environment
- Ability to work in a remote environment (Hybrid position)
- Desire to learn and grow within the oncology space
- Nursing licensure in the state of Texas or compact state
Strong Candidates Will:
- Have excellent written and verbal communication skills
- Be strong in communication with patients/Members
- Have a knowledge of insurance and TPAs
- Have oncology or infusion experience (not required)
