Job Summary:
The Care Coordinator serves as a vital link between patients, families, and interdisciplinary care teams, ensuring seamless coordination of services and continuity of care. This role requires strong multitasking abilities, proficiency in electronic medical record (EMR/EHR) systems, and the ability to effectively manage multiple systems simultaneously in a fast-paced healthcare environment. The Care Coordinator supports patients with complex and chronic conditions by facilitating access to care, addressing barriers, and promoting positive health outcomes.
PLEASE NOTE: THIS IS A FULL-TIME, IN-OFFICE POSITION.
Key Responsibilities:
- Serve as the primary liaison between patients, families, and interdisciplinary care teams to ensure clear and consistent communication.
- Coordinate, schedule, and track patient appointments, follow-ups, and referrals to specialists to ensure timely care delivery.
- Process and manage referrals, including sending orders and referrals following provider appointments.
- Maintain and manage assigned patient caseloads, ensuring all interactions are accurately documented.
- Utilize Electronic Medical Record (EMR/EHR) systems to maintain accurate, detailed, and confidential patient records.
- Demonstrate strong computer proficiency, including the ability to multitask and toggle between multiple systems and platforms efficiently.
- Review provider documentation and progress notes for chronically ill patients and communicate recommendations to appropriate clinicians.
- Develop and maintain individualized patient care plans and provide ongoing care management and coordination services using the organization’s EMR system.
- Identify and address barriers to care, such as transportation, financial limitations, or access to services.
- Connect patients and families with community resources, social services, and support programs.
- Facilitate smooth transitions of care between hospitals, specialists, home health services, and other care settings.
- Maintain frequent communication with patients and families to provide care coordination, education, and support.
- Monitor patient adherence to care plans and support compliance with palliative care programs to improve overall health outcomes
Qualifications:
- Experience in care coordination, case management, or a related healthcare role preferred
- Strong knowledge and hands-on experience with EMR/EHR systems
- Excellent multitasking and organizational skills in a fast-paced environment
- Ability to navigate and switch between multiple systems efficiently
- Strong communication and interpersonal skills
- Knowledge of community resources and healthcare care transitions is a plus
- Experience working with chronic or palliative care populations preferred

Starting Pay Rate $18.00 per hour
