Job Description:
Ennoble Care is seeking a full-time Care Coordinator, Durable Medical Equipment, Referrals, and Medical Documentation to join our Departments Team in Marlton, NJ. The Care Coordinator serves as a central point of contact for coordinating patient care by managing referrals, durable medical equipment (DME), medical documentation, insurance-related requirements, and communication between patients, providers, vendors, and healthcare facilities. This position ensures patients receive timely services while maintaining accurate documentation and supporting continuity of care.
Responsibilities:
- Coordinate all aspects of patient care, including referrals, durable medical equipment (DME), medical documentation, and related administrative workflows.
- Receive, review, and process physician orders to ensure all required clinical documentation, prescriptions, signatures, and insurance information are complete before submission.
- Communicate with patients, caregivers, clinicians, specialists, referral offices, healthcare facilities, insurance representatives, and DME vendors to coordinate services and resolve issues.
- Submit referrals, medical documentation, and DME orders through the electronic medical record (EMR) and applicable vendor or provider portals.
- Request, obtain, review, upload, organize, and maintain medical records and supporting documentation within the EMR while ensuring accuracy and HIPAA compliance.
- Monitor referral, documentation, and DME orders from initiation through completion, following up on outstanding requests, delayed responses, denials, incomplete documentation, and delivery status.
- Assist with insurance verification, prior authorization requirements, and supporting documentation as needed to facilitate patient services.
- Schedule and coordinate patient appointments with specialists, imaging centers, community providers, and other healthcare resources while ensuring consultation reports are received and documented.
- Maintain accurate documentation of all communications, patient interactions, status updates, and completed tasks within the EMR.
- Identify urgent patient needs, barriers to care, or safety concerns and promptly escalate issues to the Team Lead or appropriate clinician.
- Collaborate with interdisciplinary teams to ensure seamless coordination of care and positive patient outcomes.
- Follow departmental workflows, company policies, HIPAA regulations, and documentation standards.
- Support department operations and perform additional care coordination duties as assigned.
Qualifications:
- Previous experience in healthcare, care coordination, medical office administration, referrals, DME, medical records, insurance verification, prior authorizations, or customer service preferred.
- Working knowledge of electronic medical records (EMR), HIPAA, medical terminology, and healthcare documentation.
- Strong organizational, multitasking, communication, and problem-solving skills.
- Ability to manage multiple priorities while maintaining accuracy in a fast-paced healthcare environment.
- Comfortable spending a significant portion of the workday on the telephone.
- Proficient with Microsoft Office and standard office equipment.
- Positive attitude, adaptability, professionalism, and strong customer service skills.
- Must successfully pass a background check.
- Must be available Monday through Friday, 8:30 a.m. to 5:00 p.m. This is a full-time, on-site position in Marlton, NJ.
