ABOUT EQUUM MEDICAL

Join Equum Medical as a Telehealth Clinical Resource Nurse, you will serve as a centralized clinical resource supporting hospitals and healthcare partners nationwide. This role combines advanced clinical judgment, real-time patient surveillance, interdisciplinary care coordination, and staff support to improve patient care outcomes across multiple care settings.

Unlike traditional bedside nursing, this position focuses on proactive patient monitoring, clinical escalation, workflow optimization, and collaboration with on-site teams through Equum's virtual care platform.

As a Telehealth Clinical Resource Nurse, you will serve as a centralized clinical resource supporting hospitals and healthcare partners nationwide. This role combines advanced clinical judgment, real-time patient surveillance, interdisciplinary care coordination, and staff support to improve patient care outcomes across multiple care settings.

KEY RESPONSIBILITIES

  • Monitor patient status remotely using advanced telehealth technology and electronic health record systems (Epic, Cerner, Meditech).
  • Identify changes in patient condition and facilitate appropriate clinical escalation and intervention.
  • Support bedside clinicians through real-time clinical consultation and workflow guidance.
  • Coordinate communication among physicians, nurses, case managers, and other members of the care team.
  • Assist with adherence to evidence-based practices, quality initiatives, and patient safety protocols.
  • Document clinical activities and recommendations within designated electronic health record systems.
  • Participate in multidisciplinary rounds and care coordination activities across partnered healthcare facilities.
  • Serve as a clinical resource for hospitals, emergency departments, intensive care units, step-down units, post-acute settings, and home-based care programs.

REQUIREMENTS

  • Active unencumbered Compact RN license
  • Active NY license required
  • Current BLS and ACLS certifications required.
  • Minimum five (5) years of acute care nursing experience.
  • Experience in ICU, step-down, emergency, or progressive care environments strongly preferred.
  • Previous experience as a Charge Nurse, Preceptor, Team Lead, Clinical Coordinator, or similar leadership role preferred.
  • Experience utilizing electronic medical record systems such as Epic, Cerner, or Meditech.
  • Comfortable working in a fast-paced telehealth and technology-driven environment.
