(Partner) Nurse Practitioner/Physician Assistant

Phoenix East Valley, AZ

FIND Healthcareers is a healthcare recruitment firm led by professionals who operate within the system — not outside of it. We partner with growing healthcare organizations across the U.S. to build clinical, operational, and leadership teams that improve how care is delivered.

We recruit across the full spectrum of healthcare talent — from Licensed Practical Nurses and Physicians to practice managers and operational leaders. Whether through direct hire, contract, or temp-to-hire placements, we connect candidates with environments where they can perform and organizations with people who actually move the needle.

Backed by strong relationships across healthcare networks and training programs, we don’t just fill roles — we align talent with real-world operational needs.

Nurse Practitioner / Physician Assistant — Partnership Track

Phoenix Valley, Arizona | Full-Time or Part-Time | Mobile Primary Care

Deliver meaningful care. Help shape a growing practice.

Our client is a physician-led healthcare network bringing primary and specialty care directly to older adults in assisted and independent living communities, group homes, skilled nursing facilities, and private residences.

Their multidisciplinary team includes physicians, nurse practitioners, physician assistants, and specialists working together to make care more accessible, coordinated, and personal.

Our client is seeking an experienced Nurse Practitioner or Physician Assistant who combines strong clinical judgment with an interest in helping a practice grow. This is a hands-on patient care role with an opportunity for the right candidate to take on broader clinical leadership responsibilities and explore a potential ownership pathway.

The Role

The successful candidate will manage a primary care patient panel, providing in-person visits and telehealth when clinically appropriate. The focus is on continuity of care, chronic disease management, preventive care, and timely follow-up.

Visits are organized geographically, with opportunities to see multiple patients at the same community. Travel between care locations is part of the role; scheduling aims to limit unnecessary driving.

Key Responsibilities

  • Assess, diagnose, and treat acute and chronic conditions in an older adult population.

  • Perform comprehensive examinations, preventive screenings, and wellness visits.

  • Develop individualized care plans, prescribe medications, and monitor treatment outcomes within the provider’s scope of practice.

  • Identify changes in condition and coordinate appropriate follow-up, referrals, or escalation of care.

  • Build trusted relationships with patients, families, caregivers, and facility teams.

  • Collaborate with physicians, specialists, and clinical support staff to coordinate care.

  • Complete accurate, timely documentation in the electronic health record.

  • Contribute to improvements in clinical workflows, care quality, and the patient experience.

Qualifications

  • An active Arizona Nurse Practitioner or Physician Assistant license in good standing.

  • Current national board certification and an active DEA registration.

  • Several years of relevant clinical experience, ideally in primary care, geriatrics, home-based care, or post-acute care.

  • Strong assessment skills and sound clinical judgment, including the ability to recognize when consultation or escalation is needed.

  • Confidence managing complex patients with multiple chronic conditions.

  • Clear communication, reliable follow-through, and a collaborative approach.

  • Comfort using electronic health records and telehealth technology.

  • The ability to travel to assigned patient locations.

Experience caring for older adults and bilingual proficiency are valued.

The Partnership Opportunity

For a candidate interested in more than a traditional provider position, our client offers the potential to help shape clinical services, strengthen community relationships, mentor colleagues, and support practice growth.

A potential equity or ownership arrangement may be considered for the right candidate under a separate agreement. This pathway would depend on mutual fit, demonstrated contributions, and agreed-upon terms.

Compensation & Schedule

  • Full-time and part-time opportunities.

  • Competitive compensation with productivity-based earning potential; the specific structure will be discussed during the interview process.

  • Geographic scheduling designed to support continuity of care and efficient travel.

  • Current priority areas: East and West Valley.

We don’t just place candidates — we work inside healthcare operations every day. That means we understand what actually works on the ground. If you’re looking for a role where that matters, we encourage you to apply.
