The Care Coordinator at Foresight Mental Health plays a vital role in bridging client care between our internal teams and external partners. This position serves as a liaison for partnerships requiring concierge or closed-loop care and manages referrals for Intensive Treatment Services (IOP and PHP). The Care Coordinator works directly with referring providers, clinical leadership, clients, and external organizations to ensure timely and appropriate placement, enhance care continuity, and promote successful engagement in higher levels of care when clinically indicated. This position also provides additional support for psychiatry and therapy operations teams as needed. This individual must be highly organized, detail-oriented, and skilled in communication and care coordination within behavioral healthcare settings.
As a Care Coordinator, you will:
- Work a full-time, remote hourly position with a schedule Monday-Friday 9AM-6PM PST
- Serve as the primary point of contact for partnerships that require concierge care or closed-loop referral follow-up
- Facilitate internal and external referrals for Intensive Treatment Services (IOP and PHP)
- Track, manage, and document the lifecycle of all referrals to ensure timely outreach, placement, and follow-up
- Maintain strong working knowledge of internal service offerings, capacity, and eligibility requirements for IOP/PHP programs
- Communicate directly with clients to assist in placement or next steps in the care journey, including external resource referrals when appropriate
- Coordinate with Foresight therapists, psychiatry providers, and leadership to understand client needs and recommend appropriate level of care
- Develop and maintain relationships with external referral sources, ensuring clear expectations and consistent feedback loops
- Use judgment, organizational protocols, and supervision to guide referrals and coordination decisions
- Maintain accurate documentation within the electronic health record (EHR) and designated tracking systems
- Participate in team meetings, quality improvement efforts, and trainings related to care coordination and psychiatry support
- Completes tasks accurately, efficiently, and in a timely manner

What We're Looking For:
- High school diploma or equivalent required
- Associate’s degree in a healthcare-related field (e.g., medical assisting, behavioral health, nursing) preferred
- Minimum 2–3 years of experience in care coordination or referral management within behavioral healthcare or community health setting
- Familiarity with higher levels of care (IOP, PHP, inpatient, etc.) and behavioral health systems of care
- Experience working with electronic health records and case tracking systems
- Experience working remotely and/or with virtual care models is a plus

- Proficiency in Microsoft Office Suite, Google Workspace, and EHR platforms
- Ability to work independently in a remote environment while collaborating effectively across departments
- Demonstrated commitment to maintaining confidentiality and HIPAA compliance
- Detail-oriented mindset with a focus on quality and accountability
