Who we're looking for:
A passionate, dedicated, sharp mental health provider who wants to be part of a growing, collaborative team. As a Clinician, you will be providing best in class mental health services that will be delivered in person, designed specifically to treat addiction and mental health conditions for the populations we serve. This position sits in our Rego Park, Queens office and reports directly to our Executive Clinical Director. Please note, this is a full-time, in-person role - 40 hours a week, Monday-Friday flexible schedule with 2 evenings (12-9pm) required.
What you'll be doing:
- Provide individual counseling as needed
- Completes assessment and intake as scheduled
- Designs and implements treatment plans
- Responsible for documentation of all services provided
- Maintains contact with referral sources, including EAP's and Criminal Justice referral sources
- Generates and sends Referral Progress Reports
- Participates in weekly Case Conference meeting for treatment review of patients
- Attends weekly supervision
- Participates in staff meetings
- Assures that KPC’s policies and procedures and federal regulations regarding confidentiality of the patient and their records are strictly adhered to
- Perform other duties as assigned which include but not limited to managed care reviews, care coordination
What you'll need:
- Valid, unrestricted license in New York as a LCSW or similar credentials including LMSW, LMHC, or LMFT.
- Masters Degree in Social Work or a similar field
