Who we're looking for:
An independent, forward thinking, and mission-driven Clinical professional who wants to be part of a collaborative team between Forge Health and the Tunnel to Towers Foundation. The mission of the Tunnel to Towers Foundation is to honor the sacrifice of firefighter Stephen Siller who laid down his life to save others on September 11, 2001. Aligned with our core community at Forge, the Foundation also honors our military and first responders who continue to make the supreme sacrifice of life and limb for our country.
As the Veterans Services Licensed Clinician, you will provide behavioral health services to the Tunnel to Towers Foundation's Atlanta Veterans Village, which provides permanent affordable housing and comprehensive supportive services to men and women veterans who were previously struggling with homelessness. This is an in-person, unique opportunity for a clinician who thrives working in a robust outpatient setting providing individual services, identifying potential groups, conducting intakes, and working collectively alongside the T2T care team.
Please note, the anticipated start date for this opportunity is slated for June 2025.
What you'll be doing:
- Maintain a caseload mixed with complex Behavioral Health cases
- Participate as a member of a multidisciplinary team assigned to the client and participate in case management meetings.
- Completion of Initial Assessments for clients, as well as assessing for appropriate level of care.
- Conduct workshops, trainings, and provide other educational resources as the clinical expert.
- Obtain and review records relevant to the current treatment episode, and contact to relevant referral sources, family or outside systems as needed.
- Prepare (as needed) and maintain treatment plan and necessary revisions, in conjunction with the case manager.
- Coordinate with discharge planner regarding continuing care and recommendations for discharge.
- Documentation of all counseling services in clinical records.
- Maintain all necessary logs required for administrative tracking.
- Complete relevant state-required work for clients on your caseload.
- Communicate with family members, attorneys, EAPS, doctors or other outside resources, as needed, within provision of consent to release documents, and document all communications.
- Initiate releases, contracts for safety or any other necessary documents as needed.
- Provide crisis intervention and other functions as needed.
- Perform other duties as assigned.
What you'll need:
- Valid, unrestricted license in Georgia as a LCSW or LPC
- Masters or Doctorate Degree in Social Work or a similar field
- Demonstrated knowledge working with multidisciplinary teams
