Senior Full Stack Software Engineer - AI/LLM - Remote (EST timezone preferred)

Remote

Obesity impacts over 40% of the U.S. adult population today and is projected to affect more than 50% by 2030. Historically, only about 1% of patients have received appropriate medical treatment for this condition, but the field of obesity medicine is now entering a period of rapid growth. Form Health is on a mission to provide compassionate, accessible, evidence-based clinical care, ending the biology vs. willpower debate. Our virtual clinic ensures that science-based treatments—including GLP-1 medications—are accessible and used effectively to deliver sustainable outcomes at manageable costs for patients, employers, and healthcare plans.

Form Health patients work one-on-one with an obesity medicine doctor and registered dietitian to achieve their health goals. The Form Care Team creates a personalized plan that addresses lifestyle changes and, where appropriate, includes a prescription for FDA-approved medications. All care is delivered virtually, making it accessible to everyone. Patients use the Form app to connect with their Care Team through video visits, send messages, track their progress, and access educational resources. We are covered by most major insurance plans, including Medicare.

We are looking for an experienced full stack engineer that is excited to build iterative, AI/LLM-driven experiences for our internal users (practice managers, dietitians, providers, care coordinators), and eventually our patients. This role is critical to scaling the number of patients we serve 10x year-over-year, enhancing both the patient and operational experience. You will partner closely with product, engineering, and cross-functional stakeholders to meet user needs, and work iteratively to deliver impactful solutions.

What You Will Do

  • Build, iterate, and maintain infrastructure (AWS), API services (REST, GraphQL), data modeling (PostgreSQL), and integrations that empower our AI/LLM-driven experiences.
  • Take ownership of delivering impactful AI solutions, bringing promising prototypes into full production.
  • Design and develop AI/LLM-driven workflows using industry-standard tools and technologies, ensuring flexibility to implement solutions best suited to the problem domain.
  • Integrate AI driven workflows  into our existing product stack  with Ruby on Rails, Sidekiq, GraphQL, and ReactJS, to support physicians, dietitians, clinical admins, schedulers, and more.
  • Collaborate with cross-functional teams to support rapid iteration of AI-driven internal tools and patient-facing experiences.

Things That Are Important to Us

  • Creating a welcoming and safe place for patients
  • Protecting patient privacy and ensuring data safety
  • Simplicity and quality in product development
  • Continuous learning, growth, and scalability
  • Minimizing waste and maximizing value in an iterative, experimental approach
  • Advancing the state of the art in telemedicine

About You

  • 5+ years of experience as a full stack engineer, building scalable back-end services and developing user-facing features
  • Strong technical skills in Ruby on Rails, ReactJS, GraphQL, and AWS
  • Experience working on cross-functional teams and partnering closely with product managers, especially in experimental, AI/LLM-driven product environments
  • Ability to work in an agile, iterative manner, continuously experimenting, learning, and improving
  • Familiarity with integrating third-party APIs and managing complex workflows across homegrown and third-party software
  • Self-driven, adaptable, and excited to work in a rapidly growing startup environment focused on compassionate healthcare

Strongly Preferred

  • Experience in healthcare, care delivery, or health insurance systems
  • Boston-based or able to maximize overlap with Eastern Time (EST) working hours

Why are you being asked to complete this form?

We are a federal contractor or subcontractor. The law requires us to provide equal employment opportunity to qualified people with disabilities. We have a goal of having at least 7% of our workers as people with disabilities. The law says we must measure our progress towards this goal. To do this, we must ask applicants and employees if they have a disability or have ever had one. People can become disabled, so we need to ask this question at least every five years.

Completing this form is voluntary, and we hope that you will choose to do so. Your answer is confidential. No one who makes hiring decisions will see it. Your decision to complete the form and your answer will not harm you in any way. If you want to learn more about the law or this form, visit the U.S. Department of Labor’s Office of Federal Contract Compliance Programs (OFCCP) website at www.dol.gov/ofccp.
