Clinical FP&A Lead
Form Health is a virtual obesity medicine clinic delivering multi-disciplinary evidence-based obesity treatment through telemedicine. Obesity impacts more than 40% of the US adult population, and although historically only about 1% of patients received medical treatment for their disease, the field of Obesity Medicine is entering a period of rapid growth. About the role:
We are seeking a results-oriented Clinical FP&A Lead to join our Finance team and serve as a strategic partner across the business, helping to shape our growth and scale efficiently. You’ll be a foundational FP&A hire, leading key activities in support of financial planning, forecasting, and performance analysis. This role will report directly to the Senior Director of Finance, but will also partner closely with the Chief Operating Officer, VP of Clinical Services, and other key stakeholders to be an influential voice and contributor in ensuring our strategic objectives and initiatives are backed by sound financial perspective and actionable insights.
What you will do:
Strategic Planning and Forecasting
- Lead internal “supply and demand” forecasting across our business lines, ensuring alignment between patient demand projections and clinical and operational capacity
- Partner with Clinical Services and Operations leaders to develop and enhance staffing models by department and role
- Translate operating models into financial models that feed into our recurring financial planning and forecasting processes
- Lead strategic projects, including those specifically targeting margin and expense opportunities, to support enterprise objectives
- Execute ad hoc expense reviews in concert with Product as we introduce efficiency tools amongst our workforce and critical components of our care delivery
Reporting and Analysis
- Partner with cross-functional stakeholders in developing measurement frameworks to better enable performance monitoring and drive efficiency amongst our operations
- Drive weekly and monthly KPI reporting, including but not limited to provider utilization, capacity, team productivity, and forecasting accuracy
- Deliver regular updates and recommendations to executive stakeholders on workforce trends and performance
- Support monthly financial close and budgeting activities as necessary
Business Partnering and Decision Support
- Serve as the finance liaison for Clinical Services & Operations, as well as other critical interfacing departments such as Sales, Client Success, Talent Acquisition, and Provider Operations
- Meet regularly with assigned departments to foster relationships, review outcomes, and problem-solve
Other
- Partner within the Finance department to identify process improvements and opportunities for deeper collaboration
- Support fundraising, M&A, and strategic initiatives as needed
About you:
- Bachelor’s degree in Finance, Accounting, Economics, or related field
- 5+ years of progressive experience in finance and analytical roles, preferably with experience in banking and FP&A, including:
- Experience with healthcare industry a strong plus
- Experience in a high-growth startup or private capital-backed environment a plus
- Strong financial modeling skills and advanced proficiency in Excel/Google Sheets
- Experience with BI reporting tools a plus
- Proven ability to communicate complex financial concepts to non-finance stakeholders
- Excellent communication and leadership skills, with the ability to influence cross-functional partners.
- Highly organized, detail-oriented, and capable of managing multiple priorities in a fast-paced environment.
- Hands-on mindset — willing to roll up sleeves while also thinking strategically.

Base Annual Salary
$120,000 - $140,000 USD
