Director, Care Delivery

Remote

Form Health is a virtual obesity medicine clinic delivering physician-led, evidence-based care for obesity and related cardiometabolic conditions. Obesity affects more than 40% of U.S. adults, and as GLP-1 medications reshape the field, Obesity and Cardiometabolic Medicine is in a period of rapid growth and opportunity. Form Health is at the forefront of that shift, building a clinical model that combines rigorous, personalized medical care with the technology to make it accessible at scale. Every Form Health patient works closely with a dedicated multidisciplinary Care Team, including board-certified physicians (many ABOM-certified, and endocrinologists), advanced practice providers, and Registered Dietitians who together deliver comprehensive, one-on-one treatment that includes nutrition, behavioral health, physical activity, and FDA-approved medication, if appropriate. Through our mobile app, patients engage in regular video visits along with text messaging, photo journaling, and personalized educational content. We hold ourselves to the highest standards of clinical care and to treating every individual with empathy and respect. Founded in 2019, Form Health is a venture-backed, mission-driven company partnering with leading employers and health plans to expand access to high-quality, guideline-based obesity and cardiometabolic care nationwide. Why are you being asked to complete this form?

We are a federal contractor or subcontractor. The law requires us to provide equal employment opportunity to qualified people with disabilities. We have a goal of having at least 7% of our workers as people with disabilities. The law says we must measure our progress towards this goal. To do this, we must ask applicants and employees if they have a disability or have ever had one. People can become disabled, so we need to ask this question at least every five years.

Completing this form is voluntary, and we hope that you will choose to do so. Your answer is confidential. No one who makes hiring decisions will see it. Your decision to complete the form and your answer will not harm you in any way. If you want to learn more about the law or this form, visit the U.S. Department of Labor’s Office of Federal Contract Compliance Programs (OFCCP) website at www.dol.gov/ofccp.
