Clinic Coordinator - Physical Therapist

Engage Senior Therapy, a proud member of Fusion Physical Therapy Partners, has an immediate opportunity for a compassionate and motivated Physical Therapist to join our growing team as a Clinic Coordinator across our partner assisted living communities in Dover and Durham, New Hampshire. This is a unique opportunity to combine hands-on patient care with clinical leadership on the Seacoast while making a meaningful impact in the lives of older adults every day.

About the Role

As Clinic Coordinator, you’ll lead the day-to-day operations of up to two senior living community therapy programs while maintaining your own patient caseload. This role combines clinical care, mentorship, and operational leadership to support exceptional patient outcomes, team development, and clinic performance. You’ll work closely with interdisciplinary teams, residents, families, and Engage leadership to foster a collaborative, patient-centered environment.

Your day-to-day includes:

Patient Care:

  • Provide one-on-one physical therapy services focused on improving mobility, function, safety, and quality of life for a geriatric population
  • Conduct evaluations and develop individualized treatment plans using evidence-based approaches and functional outcome measures
  • Deliver skilled interventions throughout all phases of care while monitoring patient progress and adjusting plans as needed
  • Complete timely and accurate documentation, including evaluations, daily notes, progress notes, and discharge summaries
  • Participate in interdisciplinary care meetings and maintain ongoing communication with physicians, families, referral sources, and community staff
  • Address patient, family, and caregiver concerns while partnering with leadership to support positive patient experiences and outcomes

Staff Leadership:

  • Lead and support clinical team members including PTs, PTAs, OTs, OTAs, and SLPs across assigned communities.
  • Provide mentorship, coaching, and ongoing feedback to support clinical growth and team performance.
  • Participate in hiring, onboarding, and training of new clinical staff members.
  • Conduct staff meetings and annual performance reviews while fostering a positive, collaborative work environment.
  • Serve as a mentor and clinical instructor through Engage’s New Grad Mentorship and Student Affiliation Programs.

Clinic Operations:

  • Manage clinic schedules and coordinate patient care needs in partnership with the Director of Quality Assurance.
  • Monitor clinic performance metrics including visit utilization, cancellations, referrals, and other key operational indicators.
  • Support compliance with Medicare, state, and federal regulations as well as Engage policies and procedures.
  • Oversee clinic organization, equipment, and overall facility presentation within partner communities.
  • Identify opportunities for program growth, new initiatives, and operational improvements.
  • Address and document incidents involving patients, visitors, or staff while collaborating with leadership as needed.

What You Bring

If this sounds like you, we'd love to hear from you!

  • 2+ years of experience as a Physical Therapist in a geriatric or senior living setting
  • Active (or eligibility) for state PT license in good standing 
  • Graduate of a CAPTE-accredited Physical Therapy program
  • Knowledge of clinic operations, KPI tracking, and performance metrics
  • Previous leadership, mentoring, or supervisory experience preferred
  • Proven ability to multitask, problem-solve, and foster a positive patient and team experience
  • Proficiency with Google Workspace and/or Microsoft Office programs
  • Strong communication, organizational, and interpersonal skills
  • Commitment to continuous learning, leadership, and professional growth
