About The Role
As one of Galileo's pioneering Clinical Outreach & Engagement Partners, you'll apply your clinical expertise and cultural competency to transform healthcare delivery for vulnerable populations. This role combines your passion for healthcare equity with direct community impact - helping connect underserved patients to our comprehensive primary care model (provided at no cost to patients). You'll collaborate with an interdisciplinary team of healthcare professionals while utilizing your deep understanding of social determinants of health and case management experience to develop innovative engagement strategies.

We're Looking For:
- Clinical expertise and understanding of healthcare systems (MSW or related degree preferred)
- Demonstrated experience in case management and crisis intervention
- Cultural competency and multilingual abilities (Spanish proficiency highly valued)
- Strong track record of building trust with vulnerable populations
- Experience navigating complex healthcare and social service systems
- Proven ability to develop and maintain community partnerships
- Excellence in cross-functional collaboration and professional communication
Your Role Will Include:
- Applying clinical knowledge to assess and engage patients with complex medical, behavioral health, and social needs
- Leading strategic community outreach initiatives in healthcare settings, social service organizations, and community spaces
- Developing evidence-based engagement strategies that consider social determinants of health
- Building and maintaining relationships with healthcare providers, community organizations, and social service agencies
- Contributing to the development of best practices in community health engagement
- Providing cultural and linguistic expertise to enhance patient engagement

Location & Compensation
- Based in New York, NY (field-based role)
- Compensation: $25-30/hour plus comprehensive benefits
