Be the Link Between Behavioral Health and Whole-Person Care

As a Behavioral Health Advocate, you will serve as a critical resource for our patients, helping them access essential support services and navigate complex systems of care. You will work collaboratively with our interdisciplinary team to facilitate person-centered care.

How You’ll Make an Impact
As a vital member of our interdisciplinary care team, you will:
- Engage and Support Patients – Travel to patients’ homes, shelters, hospitals, and other community settings to build meaningful, trust-based relationships.
- Conduct Community-Based Outreach – Locate and engage high-acuity patients, ensuring they receive the care and resources they need.
- Coordinate Comprehensive Care – Assist with care transitions, including discharge planning from acute and alternative care settings, to ensure continuity of care.
- Connect Patients to Critical Resources – Act as a bridge between patients and essential community services, including housing support, legal aid, and behavioral health providers.
- Assess Needs and Facilitate Referrals – Identify patients’ behavioral and physical health needs, making appropriate referrals for services and interventions.
- Document and Optimize Care Plans – Maintain thorough, compliant records that support high-quality, coordinated care.
Who You Are
You are a passionate and resourceful advocate who believes in whole-person care and health equity. You are energized by direct patient interaction, adept at problem-solving, and driven by the opportunity to improve the system for those who need it most.
We’d love to hear from you if you have:
- A Bachelor’s degree in a related field with 2+ years of experience in care coordination/case management for individuals with chronic SMI/SUDs, OR a Master’s degree in a related field with at least 1 year of experience.
- Experience working within multidisciplinary care teams and collaborating across providers, social workers, and community partners.
- A deep understanding of social benefits programs, behavioral health resources, and community-based services.
- Strong communication skills and the ability to build trust with diverse patient populations.
- A proactive, adaptable, and solution-oriented mindset.
Where You’ll Work: A Role Rooted in the Community
We believe the best care happens where patients feel most comfortable.
- 60% Field-Based Care – Engage with patients in their homes, shelters, or other community spaces.
- 40% Remote Work – Conduct documentation, care coordination, and virtual collaboration with the care team.
- Reliable personal transportation is required (mileage reimbursement provided). Some travel will also occur using our mobile health unit.
