Position: Senior Doc Team Coordinator LVN
Location: Orange County, CA
Reports To: Director of Patient Care Services & Team Coordinator Manager
Pay Rate: $35 - $40 per hour, depending on experience

Senior Doc is looking for a Senior Doc Team Coordinator LVN to join our dynamic, caring team, where we prioritize holistic care and meaningful connections with our patients and their families.

What You’ll Be Doing:

As our Senior Doc Team Coordinator, you’ll play a critical role in ensuring seamless coordination of care for our patients. Your responsibilities will include:

  • Team Coordination: Scheduling staff, managing care plans, and ensuring effective communication among our multidisciplinary team (MDs, NPs, RNs, SWs, SCs, and more!).
  • Holistic Care: You’ll help develop and manage personalized care plans, working closely with patients, families, and specialists.
  • Documentation & Compliance: You’ll ensure accurate documentation, manage updates to care plans, and maintain compliance with care regulations and quality standards.
  • Communication & Support: You’ll serve as the communication hub, ensuring all team members are aligned and families feel heard, supported, and informed throughout their journey.
  • Cultural Sensitivity: You’ll respect diverse beliefs and practices, ensuring care plans reflect cultural preferences and enhance patient comfort.
  • Quality & Improvement: Engaging in continuous improvement initiatives, you’ll help refine care practices to maintain the highest standards of care.
