Per Diem - After Hours, Nights, Weekends
About the Role
The Licensed Vocational Nurse (LVN) delivers direct patient care under the direction of a Registered Nurse, adhering to established care plans and administering prescribed treatments. The role necessitates collaborative engagement with interdisciplinary team members to optimize patient outcomes. Specifically, this role provides on-call support for after-hours and weekend PRN visits, addressing emergent patient needs that arise outside of standard business hours. This LVN role provides on-call PRN care during off-hours and weekends, rather than maintaining a regular patient caseload.
Key Responsibilities:
- Provide direct patient care in accordance with state regulations and hospice policies.
- Implement nursing care plans based on comprehensive patient assessments.
- Accurately document patient services and update care plans as needed.
- Educate patients and families on diagnoses, treatment plans, and symptom management.
- Coordinate hospice services by reporting patient needs to the interdisciplinary team, including hospice aides, therapists, social workers, dietitians, and supervisors.
- Ensure effective and efficient use of medical equipment and supplies.
- Manage pain and symptoms, evaluating patients’ responses to treatments and medications.
- Participate in professional growth, development, and quality improvement initiatives.
- Perform other duties as assigned by the RN.

- On-Call Rate – $20 per hour for on-call triage support
- This position operates on a flat-rate beeper system at $20 per hour, whether or not calls are received. If a visit is required, the same $20 per hour rate includes the visit and mileage compensation.
What We Need From You:
- Graduate of an accredited practical nursing or vocational nursing program.
- Minimum three (3) years of nursing experience (hospice, community health, or medical/surgical experience preferred).
- Current LPN/LVN license in the state of practice.
- Strong communication, documentation, and organizational skills.
- Must have a valid driver’s license, a reliable and insured vehicle, and the ability to travel as needed.
- Current CPR certification required.
- Understanding of the hospice philosophy and the needs of terminally ill patients.
