Join Our Team as an LVN Clinical Liaison – Make a Meaningful Impact in Hospice Care!

Are you an LVN passionate about providing exceptional care and advocating for seniors? As a Hospice Clinical Liaison, you’ll play a critical role in ensuring residents and their families receive the highest level of support, compassion, and guidance throughout their healthcare journey.

What You’ll Do:

  • Be the Go-To Expert: Serve as the primary point of contact for residents, families, medical professionals, and hospice providers.

  • Facilitate Transitions: Ensure smooth transitions from hospital to hospice or home care, making the process seamless and stress-free.

  • Educate & Empower: Provide essential education to families, helping them navigate the complexities of hospice care.

  • Coordinate Personalized Care Plans: Collaborate with wellness directors, families, and hospice agencies to develop tailored care plans that prioritize comfort and dignity.

  • Advocate for Residents: Ensure each individual’s needs, preferences, and comfort remain at the heart of every decision.

  • Ensure Compliance & Quality Care: Maintain accurate records, ensure regulatory compliance, and provide ongoing support to care teams.

What We’re Looking For:

  • Licensed Vocational Nurse (LVN) (RN preferred)

  • Strong Organizational & Problem-Solving Skills – Ability to manage multiple tasks and maintain attention to detail.

  • Empathetic Communicator – Excellent interpersonal and communication skills to support families and healthcare teams.

  • Passion for Senior & Hospice Care – A deep understanding of the healthcare system and a commitment to compassionate service.

  • Experience in Operations & Compliance (a plus)

Performance Success Metrics:

  • Smooth implementation of personalized care plans

  • Strong, trusting relationships with residents, families, and care teams

  • Adherence to company standards, policies, and regulatory guidelines

Work Environment:

  • Dynamic mix of sitting, standing, walking, and occasional lifting (up to 50 lbs.)
