Hospice Registered Nurse Case Manager
Lead with Compassion & Expertise in Hospice Care!
Are you an experienced Hospice Registered Nurse who thrives in a leadership role while providing exceptional, compassionate care? We are seeking a dedicated Hospice RN Case Manager to guide and support patients, families, and interdisciplinary teams through end-of-life care. If you excel at managing complex cases, leading care teams, and ensuring the highest quality of hospice services, we want you on our team!
Key Responsibilities
๐น Lead Patient Care โ Serve as the primary RN for your patients, ensuring they receive expert hospice care with dignity and comfort.
๐น Comprehensive Assessments โ Conduct initial and ongoing assessments to develop and modify individualized care plans.
๐น Interdisciplinary Collaboration โ Lead and coordinate care with physicians, social workers, chaplains, and the healthcare team to ensure holistic support.
๐น Care Plan Leadership โ Oversee the implementation and adjustments of care plans, proactively addressing patient needs and concerns.
๐น Family Education & Support โ Empower families with knowledge and guidance to manage care at home, providing emotional support during critical times.
๐น Skilled Nursing & Symptom Management โ Administer medications, monitor symptoms, and deliver expert palliative care in home settings.
๐น Case Manager Leadership โ Take ownership of patient caseloads, ensuring seamless care coordination and acting as a mentor to fellow nurses.
๐น Regulatory & Documentation Excellence โ Maintain compliance with hospice regulations and complete accurate, timely documentation within 24 hours of visits.
Qualifications & Requirements
โ Education & Experience:
- Minimum of 1 year of hospice nursing experience (required).
- Graduate from an accredited nursing program (BSN preferred).
- Strong background in leadership, case management, or team coordination.
โ Licenses & Certifications:
- Current Registered Nurse (RN) license in CA.
- Valid driverโs license with reliable, insured transportation for home visits.
- Active BLS certification (American Heart Association or Red Cross).
โ Skills & Competencies:
- Proven ability to lead and mentor within a hospice care setting.
- Strong decision-making, problem-solving, and critical thinking skills.
- Excellent communication and interpersonal skills to support patients, families, and teams.
- Ability to work independently and collaboratively in a fast-paced environment.
- Proficiency with electronic documentation and medical records systems.
Work Environment & Physical Requirements
๐ Travel to patient homes required (1-10 visits per day) in varying weather conditions.
๐ช Ability to lift up to 50 lbs and perform standard nursing tasks.
๐ฅ๏ธ Comfortable using a tablet/laptop for documentation and patient records.
