About the Role:
As our Associate Hospice Medical Director (MD/DO), you will serve as a clinical leader within our hospice and palliative care team—providing direct care, overseeing the medical components of care planning, and collaborating across disciplines to ensure best-in-class service delivery.
Core Components of Palliative Care You'll Lead:

Comprehensive Symptom Management: Address pain, dyspnea, nausea, anxiety, delirium, and other complex symptoms in patients with serious illness.

Goals of Care Conversations & Advance Care Planning: Guide patients and families in understanding prognosis, making informed choices, and aligning treatment plans with their values.

Psychosocial and Spiritual Support: Partner with social workers and chaplains to address emotional, cultural, and existential aspects of care.

Interdisciplinary Collaboration: Work closely with nursing, case management, and ancillary teams to coordinate care across settings and ensure continuity.

Prognostication & Care Transitions: Provide input on prognosis, hospice eligibility, and timely transitions to appropriate levels of care.

Family and Caregiver Engagement: Support caregiver education, grief preparation, and continuity of care beyond the patient’s immediate medical needs.
Key Responsibilities:

Collaborate with the hospice interdisciplinary group (IDG) to develop and monitor patient-centered care plans.

Certify terminal diagnoses, conduct face-to-face visits, and participate in utilization review.

Provide palliative consults and primary palliative care oversight for patients with serious illness, even outside of hospice eligibility.

Mentor providers and staff in best practices across symptom control, communication, and ethical decision-making.

Participate in quality improvement initiatives, compliance efforts, and program development.

Serve as a liaison with referring physicians, SNF/AL communities, and external partners.
What We’re Looking For:

MD or DO, with board certification in Hospice & Palliative Medicine (or significant hospice/palliative care experience).

Active California medical license (or ability to obtain one).

At least 3–5 years of hospice or palliative care experience, with team leadership strongly preferred.

Knowledge of Medicare Conditions of Participation (CoPs) and state/federal hospice regulations.

A collaborative, patient-first approach grounded in empathy, clinical excellence, and integrity.
