The Hospice High Acuity RN provides compassionate, specialized nursing to patients nearing the end of life, who have clinically complex conditions that require a nurse who can provide specialized care while rapidly adapting to changing patient needs.  Focusing on comfort, dignity, and quality, they assess patients’ needs, develop care plans, manage care plans and offer emotional and spiritual support.

 

Key Responsibilities:

Patient Assessment and Care Planning

  • Assess patients’ physical, emotional, and spiritual needs to develop individualized care plans that address their specific needs.
  • Analyze patient situations, prioritize interventions and make decisions based on patient needs in consultation with the medical director.

Symptom Management

  • Manage pain and other distressing symptoms through medication, treatments, and non-pharmacological interventions.
  • Ability to assess patient needs for advanced airway management, ventilator management, or wound care.

Medication Administration

  • Administer prescribed medications, ensuring proper dosage and monitoring for side effects.

Emotional and Spiritual Support & Patient Advocacy

  • Provide emotional support, counseling, and guidance to patients and families, helping them navigate grief, loss, and end of life issues.
  • Advocate for patients’ wishes and ensure their needs are met, working closely with families and the healthcare team.

Education and Counseling

  • Educate patients and families about end-of-life care, advanced directives, and other relevant topics.
  • Ability to educate family/caregiver on various care management interventions.

Coordination of Care

  • Collaborate with other healthcare professionals (doctors, clinical leadership, social workers, chaplains, case managers, etc.) to ensure that the complex medical needs of the patient are fully addressed and coordinated with each discipline.
  • Coordination of all specialized equipment, medication and transportation of each high acuity patient in collaboration with hospital discharge planners, physicians, clinical leadership and the hospice interdisciplinary team.

Documentation

  • Meticulous documentation of patient care, assessments, interventions, responses and care plans.

QUALIFICATIONS

  • Must be a registered nurse (RN) licensed by the California Board of Registered Nursing (BRN).
  • Minimum of 1-2 years of recent RN experience in an acute care setting, such as ICU, TCU or ER.
  • BLS, ACLS Certification. CHPN certification a PLUS.
  • 1-2 years of Hospice Nursing experience.
  • Proficiency in handling complex cases, including vent, trachesotomy care, Airvo high flow and trauma.
  • BSN Preferred.
