Givens Highland Farms, a nationally recognized nonprofit life plan community (continuing care retirement community), is hiring a full-time Social Worker / Navigator to help support our Independent Living (IL) residents. Reporting to the Resident Services Director, the primary purpose of this position is to support the attainment and maintenance of the highest and best quality of life possible for independent living residents by providing referrals to community agencies for services not provided by Givens, and by individually counseling residents, caregivers and family members as appropriate.
What you'll do:
- Ensure the psychosocial needs of residents are met by implementing and evaluating planned social services interventions, both on group levels and in one-on-one settings.
- Support Highland Farms' resident satisfaction goals by serving as liaison between residents, other departments, and community resources
- Ensure that residents receive the optimal level of services by screening, assessing, and monitoring all residents in independent living
- Actively participate, provide input, and support department and campus-wide meetings
- Ensure that residents are in the appropriate level of care
- Perform any other duties and/or tasks that may be assigned on an as-needed basis by an authorized supervisor
What you'll need:
- Bachelor's in Social Work, experience strongly preferred
- Five (5) or more years in a human service field in long-term care, CCRC, Assisted Living, or equivalent facility and experience
- Ability to work nights and weekends as needed
- Database, spreadsheet, and Microsoft Office experience
- Working knowledge of the health care system and levels of care
- Ability to work independently, proactively, and with teams as required
- Excellent Communication, interpersonal and organizational skills
Compensations is $27.00 - $32.00 per hour, and includes our comprehensive benefits package!
