
Sr. Director, Population Health and Documentation

Role Scope:
We are looking for a Senior Director, Population Health & Documentation that can:
- Design, implement, and run the risk adjustment/burden of illness program at Habitat Health, including the suspecting, pre-charting, documentation, and coding sub-functions.
- Develop strategies around prospective, concurrent, and retrospective risk adjustment review programs and integration to drive accurate coding and burden of illness capture.
- Collaborate and coordinate with leadership and cross-functional teams across the organization, including Compliance, Practice & Center Operations, Clinical, Health Plan, and Data/Technology to drive and support a comprehensive and cohesive risk adjustment/burden of illness program that is participant and provider focused.
- Own and manage strategy for vendor relationships (including performance management and process improvements) to increase the quality, efficiencies, and effectiveness of the risk adjustment/burden of illness strategies and initiatives.
- Design, implement, and run new population health functions and/or programs (e.g., after-hours care, home care, mobile clinic, care programs/interventions, etc.).
- Understand current and future needs, skillsets, and capabilities of the various population health functions being managed, and works with leadership to strategically recruit, hire, and develop talent.
- Lead and implement performance analytics with an aim to identify areas of opportunity, key drivers of success, process optimization, and overall performance for population health related activities.
Qualifications:
- Aligns with our purpose and our values, and is excited about living those out in daily practice
- Ability to lead with influence, effective communicator, and strong interpersonal skills
- Master’s degree preferred (MBA, MHA, etc.)
- Minimum eight (8) years’ experience in a combination of risk adjustment, quality, population health, and provider engagement, with at least 4 years in a senior leadership role
- Experience with strategy development, execution, planning, and management of high priority/high visibility projects
- Proven track record of developing and implementing successful risk adjustment and population health functions, initiatives, and processes – including conducting vendor discovery, evaluation, implementation, and management
- In-depth knowledge of Risk Adjustment processes and regulations
- Excellent leadership and team management skills with a history of developing high-performing teams
Compensation:
We take into account an individual’s qualifications, skillset, and experience in determining final salary. This role is eligible for medical/dental/vision insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, paid time off, and company-paid holidays. The expected salary range for this position is $174,000 - $210,000 and is a bonus-eligible bonus. The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location.
