Role Scope:
Habitat Health is seeking an organized and compassionate Intake Coordinator to guide prospective participants through the PACE enrollment journey. This on-site role focuses on coordinating home visits, supporting additional assessments and leading center tours to help enroll new participants. Intake Coordinators will also help ensure a smooth transition to the Interdisciplinary Team (IDT). The Intake Coordinator plays a key role in facilitating the enrollment process for prospective participants, as well as connecting outreach and clinical teams while delivering an excellent experience for older adults and their families.
Core Responsibilities & Expectations for the Role
- Work with cross-functional teams to effectively enroll prospective participants, including tracking of the prospective participant end-to-end enrollment process.
- Schedule and coordinate home visits and follow-up appointments for prospective PACE participants.
- Communicate daily status of prospective participants to outreach & enrollment team to ensure timely enrollment.
- Collaborate with clinical teams to identify and facilitate any additional assessments needed prior to IDT presentation.
- Serve as a welcoming guide during center tours, educating prospects and their families about the PACE model and services.
- Track and manage leads through the enrollment funnel using a Customer Relationship Management (CRM) system and Electronic Medical Record (EMR) system.
- Manage prospective participant EMR record and prospective participant enrollment requirements needed at each stage of the process.
- Ensure timely and high-quality participant handoffs and a coordinated transition to the IDT.
- Support on-site events and activities that generate interest and promote program awareness, including support in conducting enrollment conferences.
- Maintain compliance with all federal, state, and Habitat Health policies and procedures.
- Other duties as assigned.
Required Qualifications:
- Experience in healthcare or enrollment sales, with a track record of generating business-business or business-to-customer referrals
- Experience working with State/County eligibilities workers
- Proficiency in CRM software preferred
- Experiences working with Frail Elderly
- Highly motivated individual with minimum supervision
- B.S./B.A. Degree in healthcare, Business Administration, or Marketing preferred
- Must be able to lift and carry or otherwise move up to 25 pounds
- Proof of current CPR/BLS certification required or requirement to obtain within 30 days of employment.
- Proof of valid CA driver’s license, personal transportation, good driving record and auto insurance as required by State law.
- Must be medically cleared for communicable diseases and current on all immunizations prior to participant interaction (including TB clearance).
Preferred Qualifications:
- Bilingual: Spanish/Mandarin/Cantonese preferred.
Compensation:
We take into account an individual’s qualifications, skillset, and experience in determining final salary. This role is eligible for medical/dental/vision insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, paid time off, and company-paid holidays. The expected salary range for this position is $22 - $28 per hour and is bonus eligible. The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location.
