Role Scope:
The Senior Director of Growth Operations will be the first dedicated leader focused exclusively on scaling and overseeing the day-to-day execution of participant growth across our centers. This individual will report directly to the Vice President of Growth and take full ownership of how growth is operationalized in the field. This individual will have direct leadership responsibility of Outreach and Enrollment teams, and will work collaboratively with Center Executive Directors, Medical Directors, the Clinical Pre-Enrollment team, and the central Growth team to ensure we consistently and reliably reach and exceed participant growth targets. This role will lead all aspects of field growth execution: coaching front-line staff, managing workflows and structures, improving enrollment performance, and helping design what sustainable, mission-aligned growth looks like at-scale. This role is critical to ensuring sustainable, high-integrity growth as we expand access to PACE for more communities.

Core Responsibilities & Expectations for the Role
- Serve as the senior-most field-facing growth leader, accountable for participant growth targets and performance across multiple centers.
- Manage and develop all non-clinical Outreach and Enrollment staff across Habitat Health centers. Includes setting and managing to performance expectations, providing ongoing coaching and development, and building a unified, mission-aligned growth team culture.
- Manage and develop Ops Quality and Excellence department to provide resources, skill training, and maintain accountability for successful community outreach, engagement of new prospects, and enrollment opportunities.
- Implement sustainable growth operations at the center level, including referral engagement, community outreach, prospective participant case management, and participant handoffs to the PACE center interdisciplinary team.
- Work cross-functionally with clinical enrollment teams, including the Pre-Enrollment Clinical Director and Enrollment RNs to assess PACE eligibility and submit DHCS level of care applications for prospective participants.
- Regularly review team performance data, identify gaps or bottlenecks, and implement solutions in partnership with local and central teams.
- Build the playbook and relentlessly refine for growth-focused operational execution at Habitat—developing systems and standards that can scale as we expand to new markets.
- Partner closely with Center Executive Directors and Medical Directors to align outreach and enrollment activities with clinical and operational capacity.
- Represent the Outreach and Enrollment team in broader growth discussions, ensuring central strategy is receiving field feedback and that teams are equipped to execute organizational priorities.
- Contribute to the future structure of the growth function, including identifying internal leaders, shaping team design, and evolving operating models.
- Ensure an exceptional experience for all prospective participants, in alignment with Habitat’s values.

Required Qualifications:
- 6–10+ years of experience in operations, healthcare, field growth, consulting, or service-based leadership roles.
- Experience making strategic decisions based on P&L performance, with a clear impact on business outcomes.
- Prior experience managing people or teams—directly or through influence—with a proven ability to coach and develop talent, including frontline staff.
- Strong problem solver who can connect high-level strategy to daily execution.
- Ability to use data and analytics to drive targeted activities and return on growth efforts.
- Humble leader who seeks feedback, gives credit, and owns outcomes.
- Strong communicator—able to align stakeholders across clinical, operational, and frontline teams.
- Thrives in fast-paced, evolving environments and builds structure where none exists

- Willingness to work in a hybrid work environment, with regular travel to Habitat Health centers.
- Proof of medical clearance for communicable diseases, including a TB test.
- Proof of all immunizations are up to date.
- Proof of valid CA driver’s license, personal transportation, good driving record and auto insurance as required by State law.
Preferred Qualifications:
- Advanced degree (e.g., MBA, MHA, MPH) in health care administration, business, or related field.
- Familiarity with the PACE program or other elderly care organizations.
- Experience working in geriatric primary care.
Compensation:
We take into account an individual’s qualifications, skillset, and experience to determine final salary. This role is eligible for medical/dental/vision insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, paid time off, and company-paid holidays. The expected base salary range for this position is $173,000 - $187,000 + bonus. The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location.
