JOB REQUIRES TO BE ON-SITE
Key Responsibilities:
• Coordinate seamless transitions for patients moving from home health to hospice care.
• Collaborate with home health clinicians to identify patients appropriate for hospice care, ensuring timely transitions.
• Educate patients and families about hospice care, addressing concerns with empathy and understanding.
• Provide on-site informational sessions to patients, families, and caregivers to explain the hospice transition process and available resources.
• Educate home health care teams about hospice eligibility and ensure staff are well-equipped to identify when hospice is the right option.
• Offer in-services and workshops to raise community awareness about hospice care and its benefits.
• Collaborate with multidisciplinary teams, including physicians, therapists, and social workers, to support patients during the transition.
• Perform comprehensive assessments of physical, emotional, and psychosocial needs during the transition process.
• Serve as a primary point of contact for patients and families, offering support, resources, and counseling.
• Maintain accurate and timely documentation of assessments and communications per healthcare regulations.
Qualifications:
• Valid Nursing License in CA.
• Minimum of 1 year of nursing experience, with preference for home health, palliative, or hospice care experience.
• Strong communication and interpersonal skills to deliver compassionate care and education.
• Ability to work both independently and as part of a multidisciplinary team.
• In-depth knowledge of hospice and end-of-life care principles.
Pay Range:
• LVN: $40 to $50/hour
• RN: $55 to $70/hour
