We are currently looking for a compassionate, organized, and team-oriented individual to fill the role of Full-Time, Part-Time, or Per Diem Occupational Therapist (OT) position for the Oakland-Richmond area, Walnut Creek-Brentwood, and Tri-Valley.

Responsibilities:
- The occupational Therapist will provide services within the scope of occupational therapy, following the plan of care individualized for the patient, and coordinating with the Interdisciplinary Team
- The Occupational Therapist will follow the plan of care; provide treatment; evaluate the patient's reaction to treatment and report any changes in his/her condition to the Physician, Case Manager, and/or Director of Nursing
- Maintain appropriate clinical records and/or notes and submit the records with organization policy.
- Be responsible for following all company policies and procedures

Requirements
- Current OT License to practice in California - Required
- Home Health: 1 year - Preferred
- Current CA Driver's License
- Posses Excellent Organizational Skills
- Coverage area Oakland-Richmond area as well as covering Walnut Creek-Brentwood, Tri-Valley
