Health Link is a leading provider of home health services in the San Francisco Bay Area. We are looking for a compassionate and organized Physical Therapist to join our team, either full-time, part-time, or Per Diem.

Responsibilities

• Provide physical therapy services based on an individualized care plan
• Evaluate and report patient progress to the healthcare team
• Maintain clinical records and follow company policies
• Supervise Physical Therapy Assistants (PTA) if assigned

Requirements

• Current California PT License
• Preferred: 1 year of home health experience
• Current CA Driver's License
