Health Link is looking for a dedicated Hospice Transition Nurse to provide expert care and unwavering support to patients and their families as they move from home health care to hospice care. Key Responsibilities:
- Facilitate Smooth Transitions: Ensure that patients moving from home health to hospice care experience a seamless, supportive transition tailored to their needs.
- Collaboration with Home Health Clinicians: Work closely with the home health team to identify patients who are appropriate for hospice care, ensuring timely and compassionate transitions.
- Empathetic Education: Provide patients and families with clear, compassionate information about hospice care, addressing their concerns with warmth and understanding.
- On-Site Information Sessions: Lead informational sessions for patients, families, and caregivers, explaining the hospice transition process and the resources available to them.
- Team Education & Awareness: Educate home health teams on hospice eligibility and the importance of recognizing when hospice is the right choice.
- Community Outreach: Organize in-services and workshops to increase community awareness about the benefits of hospice care and its role in enhancing quality of life during end-of-life.
- Collaborative Care: Partner with a multidisciplinary team—including physicians, therapists, and social workers—to ensure comprehensive support for patients and their families.
- Holistic Assessments: Conduct thorough assessments of patients' physical, emotional, and psychosocial needs, ensuring a well-rounded approach to their care during the transition.
- Primary Support Contact: Serve as a consistent and compassionate point of contact for patients and families, offering guidance, resources, and emotional support throughout the transition process.
- Accurate Documentation: Ensure timely and precise documentation of all assessments, communications, and care provided, in compliance with healthcare regulations.
Qualifications:
- Active Nursing License: Must hold a valid nursing license in California (LVN or RN).
- Experience: A minimum of one year of nursing experience required, with preference for backgrounds in home health, palliative care, or hospice care.
- Communication & Compassion: Excellent communication skills, with a deep ability to educate, comfort, and connect with patients and families.
- Team-Oriented: Ability to collaborate effectively within a multidisciplinary team and also work independently when needed.
- Expertise in Hospice Care: A strong knowledge of hospice principles, end-of-life care, and the unique needs of terminally ill patients.
Compensation & Benefits:
Hourly Pay:
- LVN: $40 - $50/hour
- RN: $55 - $70/hour
