Position Summary:
The Hospice Nurse Practitioner (NP) at Health Link Hospice plays a vital role in delivering compassionate and comprehensive hospice care. Working collaboratively with the hospice care team under the direction of the Medical Director, the NP ensures high-quality, patient-centered care for individuals with terminal illnesses.
The NP will conduct patient visits for hospice patients, both new and established, coordinating care with other hospice professionals. As an integral member of the Health Link Hospice provider staff, the NP will also perform Medicare and Medicaid "face-to-face encounters" as needed, supporting ongoing patient eligibility and care planning. Visits may take place in patients' homes, skilled nursing facilities, hospitals, or the Hospice Inpatient Unit.
Additionally, the NP will have independent decision-making responsibilities regarding patient care while maintaining access to Health Link Hospice medical directors for consultation, collaboration, or referral.

Essential Duties and Responsibilities:
The Hospice Nurse Practitioner (NP) will provide medical support and guidance to hospice patients and their families, including:
- Assessing and managing pain and symptom control for patients with advanced illness
- Evaluating terminal prognosis and collaborating with patients, families, and staff
- Ensuring care aligns with hospice guidelines and best practices
- Working with attending physicians and the interdisciplinary team to coordinate care
- Conducting visits in private homes, assisted living facilities, long-term care facilities, hospitals, and other locations
- Overseeing medication management and pharmacy formulary
- Completing Medicare-required face-to-face encounters with documentation, including:
- A clinical narrative supporting a life expectancy of six months or less
- A signed and dated face-to-face encounter attestation provided to the Medical Director
Qualifications & Skills Required:
- Proficiency in electronic medical records (EMR) and patient documentation
- Strong knowledge of hospice care and symptom management
- Understanding of Medicare "Face-to-Face Encounter" criteria
- Ability to collaborate with an interdisciplinary team and external healthcare providers
- Excellent communication, interpersonal, and leadership skills
- Ability to work with detailed and confidential patient information
- Strong organizational skills and ability to manage multiple patients effectively
- Computer proficiency required
Additional Responsibilities:
- Available for face-to-face (F2F) encounters as needed
- May be asked to assist with symptom or wound management visits after hours or on holidays at the request of the Director of Hospice or Medical Director
Minimum Qualifications:
- Current California NP license (Registered Nurse & Nurse Practitioner) in good standing
- DEA license required
- 2-4 years of experience in hospice, oncology, or pain & symptom management
- Must have a valid driver’s license and ability to travel to patient locations
