What You'll Do:

• Provide in-home speech therapy services as outlined in the individualized care plan.
• Evaluate Patient needs by collaborating with the care team to support patient progress and quality of life.
• Maintain timely, accurate clinical documentation in line with Health Link policies.

Requirements

• Current California Speech Therapist license - Required
• Valid CA driver’s license
• 1 year Home health experience preferred, but not required
• Excellent communication and time management skills
• Passionate about patient-centered care and comfortable working independently
