We are expanding our team and are seeking compassionate, organized, and team-oriented Licensed Vocational Nurse (LVNs) for multiple territories.

Available Territories & Pay Structure

We are hiring for three separate territories. Compensation is based on experience and territory assignment:

• Alameda County & West Contra Costa County
  Pay Rate: $50–$60 per hour DOE
  Coverage Area: Oakland/San Leandro/Berkeley/Richmond/San Pablo
  
  
• East Contra Costa County
  Pay Rate: $50–$60 per hour DOE
  Coverage Area: Antioch/Pittsburg/Brentwood
  
  
• TriValley Area
  Pay Rate: $50–$60 per hour DOE
  Coverage Area: Dublin/Livermore/Pleasanton/San Ramon/Danville

Responsibilities

• Implement the plan of care as initiated by the Registered Nurse
• Provide accurate, timely, and complete documentation in accordance with the plan of care
• Assess patient needs and deliver patient and family/caregiver education related to the diagnosis, treatment, and plan of care
• Participate in coordinating home health services, including identifying needs for additional disciplines (HHA, OT, PT, ST, MSW) and communicating them to the Registered Nurse and/or Clinical Supervisor
• Use equipment and supplies effectively and efficiently
• Complete online charting in Kinnser promptly and accurately
• Adhere to all company policies and procedures
• Perform additional duties as assigned

Requirements

• Current California LVN License (Required)
• Home Health Experience: 1 year (Preferred)
• Current CA Driver’s License
• Excellent organizational and communication skills
• Ability to work independently in the field
