Registered Nurse (RN) Home Health - South Bay

Job Title: Home Health Registered Nurse (South Bay) Compensation: $135,000+ Annual Salary + $10k Sign-On

• Local Territory Only: Your patients are in Santa Clara County / South Bay. You will never be asked to drive to San Francisco or the East Bay.

Financial Package We offer the stability of a salary with the upside of a performance bonus.

• Annual Salary: Starts at $135,000 per year (Increases based on experience & Case Management skills)

• Performance Bonus: Earn an additional up to 25% of your annual wage based on quality-of-care metrics.

• Overtime Eligible: Unlike many agencies, our clinicians are NOT Exempt. If you work late or take an extra shift, you get paid for it.

• Sign-On Bonus: $10,000 available for qualified candidates.

Responsibilities

• Provide skilled nursing care in accordance with the patient's individual Plan of Care.

• Collaborate with the Interdisciplinary Team (PT, OT, ST, MSW) to ensure holistic outcomes.

• Supervise Licensed Vocational Nurses (LVNs) when assigned.

• Evaluate patient progress and report changes to the Case Manager or Director of Nursing.

• Complete documentation in a timely manner using our AI-powered charting software.

Requirements

• Current California RN License.

• 1+ year of nursing experience (Home Health preferred).

• Valid Driver's License and reliable transportation.

• Compassionate, self-driven, and ready to work independently.
