At Health Link Home Health & Hospice, Physical Therapists own their caseload. You design the plan of care, manage your patients, and practice at the top of your license. Our job is to give you the tools, territory, and infrastructure to do that well then get out of your way.
We've been doing this in the Bay Area for over 20 years. That means stable referral flow, organized care coordination, and systems that actually work. You'll serve patients across San Mateo County — from Daly City and South SF down through Redwood City and beyond — with a caseload built around your geography, not against it.

Compensation & Benefits
- Base Salary: $140,000 – $190,000 (based on experience)
- Performance Bonus: Up to 25% of base salary, tied to quality metrics
- Sign-On Bonus: $15,000

What You'll Do
- Conduct PT evaluations and reassessments independently
- Develop and manage individualized plans of care
- Deliver skilled therapy targeting mobility, function, fall prevention, and independence
- Collaborate with PTAs on an occasional, well-supported basis
- Communicate with physicians and the interdisciplinary team
- Document with AI-assisted charting support
- Serve as the primary PT voice in patient care planning
Requirements
- Active California Physical Therapist license
- Valid California driver's license
- Strong clinical judgment and time management
- Home health experience preferred but strong outpatient, inpatient, or rehab PTs are encouraged to apply
