We're looking for a motivated, detail-oriented Clinical Coordinator to join our San Francisco office and help us continue delivering exceptional care to the patients and families who depend on us.

ABOUT THE ROLE

This is a fast-paced, high-impact position at the heart of our clinical operations. As a Clinical Coordinator, you'll serve as the connective tissue between our field clinicians, patients, families, and physicians — keeping care plans on track, documentation accurate, and communication seamless. WHAT YOU'LL DO

• Coordinate patient care and provide essential administrative support to field clinicians
• Track, clarify, and process medical orders, ensuring complete and accurate documentation
• Act as the primary liaison between patients, families, and physicians regarding care plans and updates
• Input orders and upload documentation into the electronic medical records system
• Assist with the onboarding and orientation of new clinical staff
• Order and monitor medical supplies to maintain inventory levels and ensure OSHA compliance
• Conduct chart audits to ensure accuracy, completeness, and regulatory compliance
• Support overall office operations and take on additional responsibilities as needed

WHAT WE'RE LOOKING FOR

• Current RN license in California (required)
• 1+ year of nursing experience; home health experience strongly preferred
• Associate degree or higher preferred
• BLS certification is a plus
• Exceptional verbal and written communication skills with the ability to manage high call volumes professionally
• Strong organizational and multitasking skills — you can juggle priorities without dropping the ball
• Comfortable with Apple products and proficient in medical scheduling and documentation software
• A service-first mindset with genuine warmth toward patients, families, and colleagues

SCHEDULE & LOCATION

• Monday – Friday, Day Shift
• On-site at our San Francisco office (remote work not available for this role)
