Position Overview

The Registered Nurse provides skilled nursing services in the home health setting including patient assessment, medication management, wound care, disease management, patient education, care coordination, and communication with physicians and interdisciplinary team members.

This role is ideal for experienced home health nurses looking for a more organized and well-supported field-based opportunity with efficient workflows and strong operational backing.

Qualifications

• Current California RN License required
• Home Health experience preferred
• Strong organizational and communication skills
• Ability to work independently in the field
• Reliable transportation and valid California Driver’s License
