We are seeking an organized, compassionate, and motivated Clinical Coordinator (LVN) to join our growing Hospice team in San Francisco. What You'll Do
As a Clinical Coordinator, you'll play a critical role in ensuring our patients receive seamless, compassionate care while supporting our clinical team and daily operations.
Patient Care Coordination
- Serve as a key liaison between patients, families, physicians, and hospice clinicians.
- Coordinate care services and assist with resolving patient care concerns.
- Support communication across the interdisciplinary team.
Clinical Documentation & Orders
- Request, track, clarify, and process physician orders.
- Upload and manage documentation within the EMR system.
- Ensure records are complete, accurate, and compliant.
- Assist with clinical documentation and workflow management.
Clinical Operations
- Support field clinicians with scheduling and care coordination.
- Monitor and maintain medical supply inventory.
- Ensure the supply room remains organized and OSHA compliant.
- Assist with audits, quality improvement initiatives, and regulatory readiness.
Team Support
- Assist with onboarding and orienting new team members.
- Collaborate with nurses, social workers, chaplains, physicians, and leadership.
- Contribute to a positive and supportive team culture.
Qualifications
Required
- Current California LVN License in good standing
- Strong communication and customer service skills
- Excellent organizational and multitasking abilities
- Ability to work independently and prioritize responsibilities
- Knowledge of medical terminology
- Proficiency with computers and electronic medical records
Preferred
- Experience in Hospice, Home Health, Palliative Care, Skilled Nursing, or Case Management
- BLS Certification
- Associate Degree or higher
- Experience coordinating patient care in a healthcare setting
